A report that is made under this Chapter shall include the following, if known:
(1) The name of the Elder or Vulnerable Adult,
(2) The nature of the suspected abuse, neglect, exploitation, or abandonment of an Elder or Vulnerable Adult, and
(3) Any evidence of the suspected abuse, neglect, exploitation, or abandonment.
