Whenever any person involved in the delivery of any health or social service on the Lummi Reservation, including but not limited to social workers, education workers, community health representatives, doctors, nurses, psychologists, psychiatrists, and counselors, or any police officer or other law enforcement officer, shall have reasonable cause to believe that a child has suffered child abuse or neglect, the person shall immediately report such incidence or cause a report to be made to the entity specified by Lummi Childrens Services. Any other person who has reasonable cause to believe that a child Title 8 (Childrens Code) LIBC Resolution #2013-131 25 has suffered child abuse or neglect may also report such incident to the proper law enforcement agency or to the entity specified by Lummi Childrens Services. In all cases where a report is required an immediate oral report shall be made by telephone or otherwise and, upon request, shall be followed by a report in writing. Such report shall contain the following information, if known and applicable:

(a) Name, address, and age of the child

(b) The name and address of the childs parents and other persons having custody of the child or being legally responsible for the child

(c) The nature and extent of the child&rsquos injury or injuries, if any

(d) The nature and extent of the neglect of the child, if any

(e) The nature and extent of the sexual abuse of the child, if any

(f) Any evidence of previous injuries, including their nature and extent and

(g) Any other information which may be helpful in establishing the cause of the child&rsquos death, injury, or injuries, and the identity of the perpetrator or perpetrators.
